This is a really interesting exploration of the situation, a great read. I think you did the right thing here, everyone is aware of the dangers of smoking while pregnant, there's no doubting she knew. Saying something is unlikely to have changed anything, and like you said, ultimately it is her choice, even if she made a poor one.
